Capeside may use or disclose to others your medical information for the purpose of providing or arranging for health care, the payments for reimbursement of the care to which we provide to you, and the related administrative activities supporting your treatment.

Capeside may be required or permitted by certain laws to use and disclose your medical information for other purposes without your consent or authorization.

Right to Inspect and Copy – In most cases, you have the right to inspect and copy your medical and billing records. To do this, you must submit your request in writing. If you request a copy of the information, Capeside may charge a fee for costs of copying and mailing. Capeside may deny your request to inspect and copy in some circumstances. Capeside may refuse to provide you access to certain psychotherapy notes or to information compiled in reasonable anticipation of, or use in, a civil criminal, or administrative proceeding.

Right to Amend – If you feel that your protected health information that Capeside maintains is incorrect or incomplete, you may ask Capeside to amend the information. To request an amendment, your request must be made in writing, and submitted to Capeside. In addition, you must provide a reason that supports s your request. Capeside may deny your request if you ask to amend information that was not created by Capeside, is not part of the medical information kept Capeside, or information which you would be permitted to inspect and copy, is not accurate and complete.

You have the right to receive a copy of our most current Notice in effect. If you have not received a copy of our current Notice, please ask the Receptionist for the most current copy.
